Please complete our annual Panorama Family Survey for Hook Jr. High School 2023-2024.  This survey is an opportunity for parents and guardians to help our school by sharing your opinions. Your opinions are important, and we are interested in hearing them. We are particularly interested in your thoughts on our school’s efforts to promote academic success and well-being for every student, regardless of race, ethnicity, disability, or other characteristics.

Answer all the questions based on your experiences at our school only. If you have students in other schools you will want to complete a survey for that school separately. Remember, we appreciate your honest opinions.  Your answers are be completely private and unknown to others.
